Cervico-facial surgery: the operations performed by Dr Delagranda

Some cervico-facial surgery procedures are performed by ENT surgeons, but also by other specialists depending on the procedure (dermatologist, maxillo-facial surgeon, oral surgeon, neurosurgeon, vascular surgeon, visceral surgeon, ophthalmologist).

Below is a summary of the cervico-facial surgery performed by Dr Antoine Delagranda, ENT surgeon in La Roche-sur-Yon in the Vendée. Please click to see the detailed page for each operation.

Neck surgery

Neck or cervical surgery is a speciality of ENT surgery which essentially covers the following procedures: thyroglossal tract cysts, cysts and fistulas of the 1st, 2nd, 3rd and 4th branchial arches, sternal cyst or fistula, bnaso palatal cyst, cystic lymphangioma, adenectomy, exploratory cervicotomy, cervical lymph node dissection, etc. The aim is to eliminate an embryonic malformation, make a histological diagnosis of a lymph node or eliminate a cancer. These surgical procedures are performed on adults and children.

Wisdom teeth surgery

Wisdom teeth removal surgery can be performed under general anaesthetic by your ENT specialist on an outpatient basis. The aim is to stop the dental pain associated with the growth of wisdom teeth, avoid tooth displacement, facilitate the work of orthodontists and reduce the risk of cavities on these molars. It is aimed at teenagers and young adults.

Vagus nerve implant for epilepsy

The vagus nerve implant is a complementary/adjuvant solution to drugs that can be proposed for disabling epilepsy that is resistant/refractory to medical treatment in children and adults (occurrence of seizures despite taking 2 different anti-epileptic drugs). The neurologist determines the indication for VNS therapy and the ENT surgeon places the vagus nerve implant required for VNS therapy. The aim is to reduce the number, frequency and severity of partial or generalised seizures and, if possible, to reduce the doses or number of drugs taken in order to improve quality of life by reducing side effects (depression, fatigue, insomnia, confusion, dizziness). The intervention concerns both children and adults.

Thyroid and parathyroid surgery

Surgery on the thyroid and parathyroid glands is a speciality of ENT and cervico-facial surgery, which essentially covers the following procedures: hemi thyroidectomy, total thyroidectomy, ¾ or 7/8th parathyroidectomy, removal of parathyrtoid adenoma. The aim is to eliminate thyroid cancer, remove a troublesome thyroid tumour or cyst, treat primary or tertiary hypercalcaemia. The aim is to prevent thyroid cancer from progressing, reduce the difficulty of swallowing (dysphagia), reduce thyroid pain, improve dyspnoea, reduce tracheal compression and avoid the complications of hypercalcaemia (heart rhythm disorders, osteoporosis, etc.). It is mainly used in adults.

Fracture of the nasal bones

Performed under general anaesthetic in the operating theatre on an outpatient basis, nasal bone surgery involves inserting a non-injurious instrument into the nasal cavity and performing gentle, gradual manual reduction of the fracture until satisfactory reduction is achieved. In both adults and children, reduction of fractures of the nasal bones is performed in cases of recent post-traumatic deviation of the nasal pyramid. The aim of the operation is twofold: to restore a satisfactory aesthetic appearance to the nasal pyramid, and to restore a sensation of freer airflow to the nasal cavity on inspiration.

Surgery on skin tumours

Cutaneous surgery of the face and neck is performed by ENT specialists. It mainly involves the removal of benign tumours: nevi or moles developed from melanocytes, keratotic lesions (seborrhoeic or actinic) developed from keratinocytes, sebaceous cysts developed from sebaceous glands, lipomas, and malignant tumours: cutaneous carcinomas or epitheliomas which develop from cells of the epidermis called keratinocytes, including basal cell carcinomas (BCC), the most common, tumours which develop slowly and essentially locally, never metastasising but can be destructive, squamous cell carcinomas (SCC) or squamous cell carcinomas (SCC), which are more aggressive, can metastasise and often arise from a precancerous lesion, and melanomas, which develop from epidermal cells called melanocytes. It mainly concerns adults, but children can also be operated on (suspected nevi, melanomas, malformations such as prehelical fistulas).
