ENT sinus surgery

Sinus surgery is a speciality of ENT surgery that covers a large number of procedures: endonasal polypectomy, ethmoidectomy, meatotomy, medial endonasal maxillectomy, dacryocystorhinostomy, balloon sinusplasty, opening of the frontal sinus, sphenoidotomy, etc.
These procedures are essentially aimed at improving nasal breathing, nasal permeability, rhinorrhoea and the sense of smell, reducing facial and sinus pain, stopping epistaxis, reducing infections (sinusitis, meningitis, sinus mycosis) and epiphora, and eliminating malignant tumours (squamous cell carcinomas, adenocarcinomas) and benign tumours (schneiderial papillomas, including inverted papillomas).
They mainly concern adults.

Imperforation of the choanae

Choanal imperforation surgery is a surgical procedure that consists of making the cavum and nasal cavities continuous by re-establishing an opening through the removal of mucous membrane and bone. It is performed exclusively on children.

Dacryocystorhinostomy

Tear sac surgery, or endonasal dacryocystorhinostomy, is a surgical procedure that involves opening the tear sac in the nose to allow tears to flow out. The operation is performed under general anaesthetic by an ENT surgeon, and generally requires a one-day stay in hospital. It is performed on adults and children. No scars are visible, as the procedure is performed through the inside of the nose.

Septoplasty

Septoplasty is a surgical operation on the median nasal septum to make it straighter, in order to improve breathing and nasal comfort. The operation is performed under general anaesthetic by an ENT surgeon, and requires a one-day stay in hospital. No scars are visible, and the procedure is performed through the inside of the nose. It is performed on adults and children aged 15 and over.

Turbinoplasty

Turbinoplasty is an endonasal surgical procedure on the lower turbinates to improve nasal permeability. No scars are visible, and the procedure is performed through the inside of the nose. It is performed on adults and children over the age of 15.

Endonasal polypectomy

Endonasal polypectomy is the removal of polyps from the nasal cavities that come from the sinuses and are the expression of nasosinus polyposis. The operation is performed under general anaesthetic on an outpatient basis by an ENT surgeon. It is performed on adults and children.

Endonasal ethmoidectomy

Adenoidectomy, or vegetation surgery, is a surgical procedure involving the removal of vegetations located in the cavum, most often in children. The operation is performed under general anaesthetic by an ENT surgeon, and requires a one-day stay in hospital.

Endonasal medial maxillectomy

Medial endonasal maxillectomy is the removal of the medial intersinonasal septum, which corresponds to the medial wall of the maxillary sinus in contact with the nasal cavity. This surgical procedure, performed on an outpatient basis by an ENT surgeon, is used almost exclusively to treat benign tumours such as schneiderian papillomas, the most common of which are inverted papillomas. It is performed on adults.

Middle endonasal meatotomy

Endonasal meatotomy is the opening of the maxillary sinus. This surgical procedure, carried out on an outpatient basis by an ENT surgeon, involves enlarging the natural drainage opening of the maxillary sinus, known as the maxillary ostium, to allow the inside of the sinus to be cleaned during the operation and to ensure that it remains clean afterwards. Infections and the presence of foreign bodies of dental origin constitute the vast majority of indications for this surgical procedure. It is mainly performed on adults.

Sphenoidotomy

Sphenoidotomy is an endonasal opening of the sphenoid sinus performed in cases of antibiotic-resistant bacterial sinusitis (sphenoiditis), mycosis, mucoceles, diagnostic biopsy and closure of a spontaneous or traumatic osteomeningeal breach. This surgical procedure, performed on an outpatient basis by an ENT surgeon, mainly concerns adults.

Frontal sinusotomy

The frontal sinuses are opened endonasally in cases of antibiotic-resistant bacterial sinusitis or mucoceles. This surgical procedure is performed by an ENT surgeon on an outpatient basis for adults.

Endonasal breach

Occasionally, a bone breach in the front of the skull causes cerebrospinal fluid to pass from the brain into the sinuses and nose, with the risk of meningitis. The passage of meningeal fluid (meningocele) or even brain fluid (encephalocele) through the breach is much rarer. Surgery consists of closing the breach through the nose with a graft.

Balloon sinusplasty

The main reasons for dilating the nasofrontal canal are conflicts leading to antibiotic-resistant bacterial sinusitis that cannot drain naturally, and problems with the atmospheric pressure balance between the nasal cavity and the sinuses. This surgical procedure, performed by an ENT surgeon on an outpatient basis, is intended for adults.

Oral-sinus communication (OSC)

The bucco-sinusal communication (BSC) is the abnormal continuity between the maxillary sinus and the oral cavity. Most of the time, this communication can close on its own, but if it persists, it creates a fistula that will require endobuccal closure surgery. This surgical procedure is performed by an ENT surgeon on an outpatient basis for adults.

Endonasal ethmoidectomy

Endonasal ethmoidectomy is a surgical procedure recommended for the treatment of naso-sinusal polyposis as a complement to medical treatment, for the treatment of ethmoid cancers (squamous cell carcinoma, adenocarcinoma) and benign tumours (schneiderial papillomas).
